What Does Health Insurance for a Senior Citizen Actually Cost in India?
Every senior-health comparison table in India puts the premiums side by side and lets you pick the small number. That comparison is meaningless, because the premiums are not for the same thing. Niva Bupa's Senior First starts you at a 50% co-payment — you pay half of every admissible claim — and ICICI Lombard's Golden Shield does exactly the same [1]. Tata AIG's Elder Care rate already has a 20% co-payment baked into it [2,3]. Put those three on one line and the cheapest headline belongs to the plan that pays the least. Match them at the same 20% co-payment, ₹10 lakh of cover, age 61–65, metro zone, and the order reverses: ICICI Lombard ₹34,409, Tata AIG ₹35,000, Niva Bupa ₹36,930 [4][2][5]. Below: that comparison in full, every age-banded rate we could read out of a filed prospectus or rate chart, and the reason one insurer's published chart is still adding 18% GST to a product that has been exempt since 22 September 2025 [6,4].

Compare the headline rates instead and Niva Bupa looks about a third cheaper than Tata AIG. That comparison prices a policy that pays half your claim against one that pays 80% of it.

1. The premium is not the price
Every senior-health page in India prints the same kind of table: insurer, plan, premium. It reads like a price comparison and it is not one, because the products in the columns have agreed to pay different fractions of your parent's hospital bill.
Niva Bupa's Senior First starts every buyer at a 50% co-payment — you bear half of each admissible claim — unless you choose a lower one when the first policy is issued, and once chosen it can never be changed [5,10]. ICICI Lombard's Golden Shield carries the identical 50% base co-payment [1,30]. That is two of the eleven private senior products filed in India defaulting the buyer to bearing half of every claim [1]. It is a market design, not one insurer's quirk, and it is what makes their headline rates look low.
Meanwhile Tata AIG's Elder Care applies a mandatory 20% to every claim and prices it in [2,3]. Star Health's Red Carpet applies 30% [31,32]. HDFC ERGO's Optima Senior applies 15% or 30% depending on the room, and then another 30% on fourteen named procedures — cataract, joint replacement, CABG, prostate surgery, the operations seniors actually have [7,33][7]. United India applies none at all [34]. Across the largest private senior products the spread runs from nothing to 50% [5,2,7,8,9].
So the first question about any senior premium is not "how much" but "how much of the claim does this number buy". Everything below is arranged around that.
2. Matched properly, the cheap plan is the dear one
Take the comparison people actually make — ₹10 lakh of cover, a 61–65-year-old, a metro address — and hold the co-payment at 20% for all three carriers that can be priced there.
Niva Bupa's published rate for that cell is ₹23,749 [5], and against Tata AIG's ₹35,000 [2] it looks like the obvious buy. But ₹23,749 is the 50%-co-payment rate. Moving to 20% is a published loading of 55.5% of base premium [5], which takes the same cover to ₹36,930 [5]. (That multiplication is ours, from the insurer's own published loading — it is flagged as computed in our ledger, not presented as a quoted rate.)
ICICI Lombard does not need the arithmetic. Its chart prices all four co-payment levels directly, and the 20% cell for that buyer is ₹34,409 [4]. For that exact buyer, moving off its 50% default costs 16.8% more to reach 40%, 33.2% to reach 30% and 49.1% to reach 20% [4] — so the ₹23,081 default becomes ₹34,409, and the numbers reconcile. (Averaged across the whole 288-cell grid the same three moves cost 17.2%, 33.7% and 49.8% [4]; use the grid figure only for market-wide statements, never beside a single cell.) It is cheaper than Niva Bupa at every step, and ICICI's co-payment can be changed at a later renewal where Niva Bupa's cannot [1][5,10].
The order at matched cost-sharing is therefore the reverse of the order on the comparison screens. That is the whole of the finding, and it is not available anywhere else because nobody else has read both rate charts.
3. What ₹5 lakh actually costs, with the cost-sharing attached
₹10 lakh is the fair comparison for the three carriers above. ₹5 lakh is what most senior products actually sell, and it is where the public-sector policies live.
₹5 lakh is the sum insured most senior products actually sell, so it is the fairest cross-market line. Read the third column before the second — it is where the difference between these numbers goes. Zones differ between insurers and are marked where the rate is zone-specific.

Two things fall out of that table. The first is that the price range at a fixed sum insured is more than two to one, and almost all of it is explained by the third column rather than by cover quality. The second is that the two cheapest lines are the two you would least want in a crisis: a 50% co-payment with a shared room and stripped benefits, and a 10%-plus-20%-plus-15% co-payment stack sitting on top of a ₹10,000 cataract cap and a surgeon's fee capped at a quarter of the sum insured [36][36,37].
New India's Senior Citizen Mediclaim is the genuinely cheap policy in this market — ₹4,447 a year at 60–65 [15,16], no co-payment [16,43], and pre-existing conditions covered after 18 months [16], against 24 at National, 36 at its own Sixty Plus and 48 at United India [39][41,16][34,42]. It is cheap because it is small: the maximum sum insured is ₹1.5 lakh [16,15], room rent is capped at 1% of that a day [16], and a cataract operation with an imported foldable lens is capped at ₹10,000 inclusive of room, theatre and surgeon [16]. As a second policy under a bigger one, it is good value. As the only policy, it is not cover.
4. Age is the price
What ageing does to the price, taken from each insurer's own bands. The last two rows are the ones to read before buying for a parent in their late seventies: the question is not what the policy costs now but whether the insurer publishes a rate at all when your parent is 88.

The pattern that matters here is not the level but the slope, and the slope is steep enough to change what "affordable" means over a fifteen-year holding period. HDFC ERGO's own chart multiplies the same ₹5 lakh policy by 4.27 between the 61–65 band and the over-85 band [7]. National's multiplies by 2.67 [38].
Those band-to-band jumps are separate published rates, not premium revisions, so IRDAI's 10%-a-year cap on senior repricing [44] does not flatten them. A family buying at 62 on the basis of today's number should look at the same chart's 76–80 line before deciding what they can sustain.
The other half of the question is whether the insurer will publish a rate at all when it matters. Tata AIG prints to a 96-plus band [2] and Universal Sompo past 90 [28]. ICICI Lombard — the carrier with the deepest rate disclosure in the country — publishes nothing above 85, on a product that advertises no maximum entry age and no maximum renewal age [4,1]. There is no published number at which an 86-year-old Golden Shield policyholder renews.
5. GST: exempt since 22 September 2025, and one chart that still adds 18%
Individual health insurance in India became exempt from GST on 22 September 2025 [11,12][11], and the GST Council's recommendation named senior-citizen policies expressly [12]. Family floaters are covered [11,45], and the definition is wide enough that a senior policy sold as a single product with embedded travel or personal-accident features is exempt in full [11,45].
Three consequences a buyer needs, in order.
Renewals count, and the test is the date of payment. A policy bought in 2019 renews exempt as long as the premium is paid on or after 22 September 2025 [45]. A policy that lapsed before the date and was revived after it pays nothing on the reinstatement premium either [45]. If a 2026 renewal notice carries an 18% line, that is an error to challenge, not a tax to pay.
The employer-floater route did not get cheaper. The most-repeated workaround in this market — put your parents on your company's group policy — sits on the one side of the exemption that was left out. Group and employer-sponsored cover still bears 18% [11,45].
And a published rate chart is not automatically current. ICICI Lombard serves two Golden Shield rate charts from the same downloads index, one marked exclusive of GST and one inclusive. Dividing the second by the first, cell by cell across all 1,152 cells, gives a ratio between 1.1799 and 1.1801 — the chart applies exactly 18% to a product that has been exempt for eleven months [6,4]. Page 2 of that same file carries two UINs stacked in its footer, the first of which the carrier's own filed product list records as withdrawn on 30 September 2024 [6,13]. A buyer who downloads it is reading rates for a product that no longer exists, grossed up at a tax that no longer applies.
It is not an isolated lapse. HDFC ERGO's Optima Senior prospectus — the only document in existence carrying that product's rates — still instructs that "Service Tax to be charged as applicable", a tax replaced by GST in July 2017 [7]. National Insurance's Varistha senior prospectus still prices the product with service tax extra [14]. The one chart in the market demonstrably produced after the exemption is Universal Sompo's, generated on 15 October 2025 — and it states no tax position at all [28].
Do not expect a clean 18% saving to persist. The move was from "18% with input tax credit" to exemption, not to a zero rate, so insurers lost the credit on their own inputs and are recovering part of it through base premium [12,11]. Analysts on Star Health's Q2 FY26 call sized full absorption at up to 3–3.5% of premium [46]; Niva Bupa reported the residual at roughly 50 basis points of combined ratio after cutting costs and renegotiating with vendors [47]. For seniors those increases are themselves capped at 10% a year [44], for anyone aged 60 and above [44].
6. Your city moves the number twice
Zone changes the premium and then changes the claim.
On the premium: Tata AIG charges a Zone A buyer about 23% more than a Zone C buyer for identical cover at the same age [2]. Niva Bupa's Zone 1 / Zone 2 gap is about 19% — and its prospectus never states which cities fall into which zone [5], so a buyer cannot check which rate applies to them. ICICI Lombard is the counter-example: it defines its zones city by city, down to the district level of the National Capital Region [1].
On the claim: buying the cheaper zone can cost more than it saves. ICICI Lombard adds up to 25% zonal co-payment when a Zone C policyholder is treated in Zone A, so a small-town buyer at the 50% default who is referred to Delhi bears 75% of the bill [1]. ManipalCigna adds 10% for a Zone II buyer treated in Zone I and 20% for a Zone III buyer [48,17]. National charges 18.5% to a Zone 2 policyholder treated in Zone 1 [39,38]. If the serious hospitals your family would actually use are in a dearer zone, buy the dearer zone.
7. Half the market publishes no price at all
The blanks are the finding. An insurer that publishes no rate is an insurer whose price you can only learn by handing over your parent's phone number.

The absences are worth as much as the numbers. Two prospectuses — ManipalCigna's and Royal Sundaram's — instruct the reader to refer to a "Rate Chart" annexure that is not attached to the document and is published nowhere else [17,18][22]. Two insurers market a senior-citizen category with no senior-citizen filing behind it: SBI General runs the page and files no senior product [23,24], and Aditya Birla's own "Health Insurance For Senior Citizen" link points at a general plan rather than at Activ Care, the senior product it actually files and documents [60,61,62].
And where a price does exist, check which document it came from. ICICI Lombard's prospectus illustrates a "Golden Shield Essentia Plan" that appears in no other Golden Shield document, at a premium its own rate chart does not contain [1,4,63]. New India publishes a superseded Senior Citizen Mediclaim prospectus in the same directory as the current one, quoting premiums about 10% below the filed rates [64,16,15]. National has two different live rate charts for products of the same name [38,52]. Oriental publishes no first-party senior premium chart at all [58,59]. A quote that matches a stale document is not evidence of anything.
8. Checking a quote before you pay it
- Ask for the co-payment in writing, as a percentage, before you look at the premium. If it is 50%, ask what the 20% version costs — 55.5% more at Niva Bupa [5], 49.1% at ICICI Lombard for a ₹10 lakh, 61–65 buyer [4].
- Ask whether the co-payment can be changed later. ICICI Lombard's can, at renewal [1]. Niva Bupa's cannot, ever [5,10]. A 61-year-old who takes 50% to keep the price down cannot buy out of it at 75, which is when it starts to matter.
- Check the tax line. Individual senior cover is exempt [11,12]; an 18% line on a 2026 renewal is wrong [45]. If your parent is on your employer's group floater instead, 18% is correct there [11,45].
- Read the sub-limits for the two operations seniors actually have. Cataract caps range from ₹10,000 an eye at Universal Sompo [36] to ₹25,000–₹75,000 by sum-insured band at ICICI Lombard [30], with joint replacement capped separately [2].
- Look up the age band your parent enters next, not the one they are in. The escalation is 2.67× to 4.27× over the life of the policy [38][7], and it is not what the 10% repricing cap governs [44].
- Ask what happens at 86. If the insurer publishes no rate above 85 [4,1], the renewal price at that age is whatever they decide it is.
