What a row is
One county, one care scenario. Each row names the cheapest plan on premium and the cheapest plan once that year’s care is paid for, with both carriers, both plan names, both deductibles, both totals, and the difference between them. Three scenarios per county — a broken arm, a year of managing type 2 diabetes, and a birth — so a 409-county file is 1,227 rows. The scenarios are CMS’s own stylised courses of care, not a forecast of anyone’s year.
