What Is in the PM-JAY Package List (HBP 2022), and What Does Each Procedure Pay?
Download the PM-JAY package list, HBP 2022 (PDF, 2.3 MB). It is the National Health Authority's Health Benefit Package Master 2022, issued with Office Memorandum S12015/008/2022/NHA(HPQA) dated 6 April 2022 [1]. The 203-page file sets what Ayushman Bharat PM-JAY pays a hospital for each procedure, in 1,949 numbered rows [1]. Every row carries four prices: a national reference price and one each for Tier 3, Tier 2 and Tier 1 cities [1]. It is still the current version. NHA's own guidelines, effective 17 April 2026, list no version after HBP 2022 [2].

1. What the document is
The PDF opens with a two-page Office Memorandum dated 6 April 2022, signed by Sraddha Paul, Deputy Director at NHA and addressed to every state's health secretary. It explains that the package master was reviewed for 2022-23 using utilisation data, cost evidence and consultation with specialty expert committees, state health agencies and hospital associations, and that the result "has been named as HBP 2022" [1]. Annexure 1 is the master itself. Annexure 2 lists 25 high-end drugs, 10 high-end procedures and 13 high-end diagnostics with prices. The last four pages name the Tier 1 and Tier 2 cities [1].
The OM also mentions an Annexure 4, the Governing Board order on state flexibility. It is not in the file.
2. Is HBP 2022 still the current package list?
Yes, as far as anything NHA has published shows. Its Revised User Guidelines on National Health Benefit Packages, effective 17 April 2026, describe five versions and stop at HBP 2022 [2], and its HBP Manual Part 2, posted in July 2026, sets out at length how a future revision would be costed and consulted on but does not announce one. Searchers often type "package list 2.2". That is the November 2021 version, with 200 fewer packages and no city-tier pricing.
From Table 1 of NHA's Revised User Guidelines on National Health Benefit Packages, effective 17 April 2026. HBP 1.0 had packages only, no separate procedures.
The procedure count is where the sources disagree slightly. This file numbers 1,949 rows, NHA's 2026 documents say 1,962 and PIB's September 2026 explainer says 1,961 [1,2,3][3]. None of the sources explains the gap, so treat the PDF as the list as issued in April 2022 and check any procedure you cannot find in it with the hospital.
3. Specialties and how the rows are organised
The rows sit under 27 two-letter specialty codes, from BM (Burns Management) to US (Unspecified Surgical Package). By our count Medical Oncology is the largest with 288 rows, followed by General Surgery (177), General Medicine (157), Orthopaedics (141) and cardiothoracic and vascular surgery (138) [1]. HBP 2022 was the version that added Palliative Medicine and bone marrow transplant procedures, and renamed Interventional Neuroradiology as Interventional Radiology [2].
Each row has a package code (SE020, cataract surgery) and a procedure code beneath it (SE020A for phaco, SE020B for SICS). The procedure code is the one the hospital books.
4. How the rates work
There are four price columns: the national reference price (NRP) and then Tier 3, Tier 2 and Tier 1 [1]. The tiers follow the 7th Pay Commission city classes. Eight cities are Tier 1, 97 are Tier 2 (Jaipur, Lucknow, Patna, Chandigarh and Indore among them), and the rest of India is Tier 3 [1].
NHA's weights add 25% for a tertiary surgical package and then a further 17% in Tier 2 or 25% in Tier 1, while medical packages get smaller steps of 10% for tertiary care and 15% for either of the bigger city tiers [2]. In the PDF that shows up simply. For a secondary procedure like cataract surgery, Tier 3 equals the NRP. For a tertiary one like knee replacement, Tier 3 already sits about 25% above it.
The final payment is the procedure price plus any stratification amount, incentive and implant [2]. Two operations in one sitting pay 100% for the dearer and 50% for the second [2].
Indicative only: each state decides whether to pay them. Calculated on the procedure rate, never on the implant, and compounded rather than added.
| Incentive under HBP 2022 | Extra over the rate |
|---|---|
| Bronze quality certification | 5% [2] |
| NABH entry level | 10% |
| Full NABH or NQAS accreditation | 15% |
| Hospital in an aspirational district | 10% |
| Teaching hospital, or metro location | Not applicable under HBP 2022 |
5. Rates for common procedures
These are copied from Annexure 1 with the PDF page, so you can check each one.
Rates in rupees, as printed in Annexure 1. NRP is the national reference price. The implant column is paid on top of the procedure price. Your state's rate can differ by up to 10% upwards, or more downwards.
| Procedure (code) | NRP | Tier 3 | Tier 2 | Tier 1 | Implant mapped | PDF page |
|---|---|---|---|---|---|---|
| Cataract, phaco with foldable IOL (SE020A) [1] | 4,500 | 4,500 | 5,300 | 5,700 | Lens 3,000 | 124 |

| Caesarean delivery (SO057A) [1] | 12,000 | 12,000 | 14,100 | 15,000 | None | 155 |
| Normal vaginal delivery (SO074A) [1] | 8,000 | 8,000 | 8,800 | 10,000 | None | 156 |
| Haemodialysis, chronic (MG072D) [1] | 1,500 | 1,500 | 1,800 | 1,800 | None | 37 |
| Appendicectomy, open or lap (SG017A/B) [1] | 20,000 | 20,000 | 23,400 | 25,000 | None | 129 |
| Cholecystectomy, open, no CBD exploration (SG039A) [1] | 31,050 | 31,100 | 36,400 | 38,900 | None | 131 |
| Inguinal hernia repair, open (SG050A) [1] | 20,000 | 20,000 | 23,400 | 25,000 | Mesh | 132 |
Medical admissions, such as fever, sepsis or poisoning, are paid per day by bed type.
Per-day medical management packages (for example MG001A, acute febrile illness, PDF p. 18). The total is the daily rate times the days in that bed category.
Two oddities are worth knowing. Several NRPs are priced to the paisa, like ₹29,062.50 for knee replacement. The haemodialysis row gives ₹1,500 without saying per what; it is most likely per session, but the row itself does not say so [1].
6. How to look up a procedure
- Open the PDF and search (Ctrl+F) for the procedure name in simple words: "cataract", "hernia", "knee". Many procedures appear under more than one specialty, so check every match.
- Note the procedure code, such as SB039A. The hospital books the procedure by that code.
- Read across to your city's tier column. If your city is not in the lists on PDF pages 200-203, it is Tier 3.
- Add the implant column if there is one. That is the national price, and your state's own package master may differ by up to 10% upwards, or more downwards [1,2].
- If your operation is not in the list at all, ask whether the hospital will book an Unspecified Surgical Package, which the state agency can approve up to ₹5 lakh [1,2].
7. What it does not tell you
It does not give your state's rates, its public-reserved list, or which hospitals near you are empanelled. Caesarean delivery is the biggest trap. It is in the master at ₹12,000 to ₹15,000, but NHA reserves it for government hospitals unless your state has opened it up [2]. The list also does not tell you how much of your family's ₹5 lakh a year has already been used, whether a particular hospital will accept the card for a particular procedure, or how the claim will be checked once you are discharged. That process is changing, with NHA moving more claims to automated adjudication, as we covered from Aarogya Manthan 2026 [4].
Our view: the master is far more useful to patients than its 203 pages of tables suggest. It is the clearest evidence you have if a hospital asks you to pay for a listed procedure.
8. Where it fits
Since 2024 the same package list applies to every Indian aged 70 and over, regardless of income, with cover of up to ₹5 lakh a year on a family basis [5][5]. How that cover works, who gets a separate top-up, and how it sits beside a private health policy you may already hold is on our government schemes for senior citizens page. It is worth reading first. For what NHA announced in September 2026 about claims and its patient data, see Aarogya Manthan 2026. The original file is on nha.gov.in.
