How Do You Fill In the LIC NEFT Mandate Form, and What Do You Attach?

By Best Worst Insurance Editorial TeamUpdated 2 October 2026

Download the LIC NEFT mandate form (PDF, 1.3 MB). It is LIC's "National Electronic Funds Transfer - Mandate Form", subject line "Receipt of Policy Payment Through NEFT": two scanned pages in Marathi, Hindi and English, print reference R.P./1 LAC Forms-11/16 [1]. You fill it in once to give LIC the bank account it should pay your policy money into, and one form covers up to six policies [1]. Our copy is the file LIC links as "12. Neft Mandate Form" on its download page, unchanged [2].

What it is forRegistering the bank account LIC pays policy money into by NEFT, for up to 6 policies [1]

1. What this form is

The NEFT mandate is LIC's bank-details form. Policy money is paid by electronic transfer, and the transfer needs somewhere to go: this form gives LIC the account number, IFS code and account type, signed by you [1]. LIC's own instructions call it the "application for NEFT" and say it can cover six policy numbers [1]. Its subject line is "Receipt of Policy Payment Through NEFT", which is why it travels with maturity, survival benefit and claim paperwork rather than with premium forms.

The copy LIC links is a 2018 scan of a printed Mumbai Division-I form, in Marathi, Hindi and English [1]. That surprises people. It is still the file LIC puts under "12. Neft Mandate Form" on its national download page [2], and the English text is the one that counts if anything is disputed. You can download it here unchanged: LIC NEFT mandate form (PDF, 1.3 MB).

2. Page by page

Page 1 is the form. A "To" block for your branch, the subject line, a one-line declaration ("I am giving below the details of my Bank account for receiving policy payment through NEFT"), then twelve numbered fields, the tick-list of enclosures, and a date and signature line at the bottom [1].

Page 2 is instructions only, given three times: in Hindi as (a) to (i), in Marathi, and in English as "Necessary information to the Policy holder/claimants opting for NEFT". Nothing on page 2 needs filling in.

3. How to fill it in, field by field

Page 1, top to bottom. Field numbers are the form's own; the policy number and name lines are unnumbered above field 2 [1].

Field on the formWhat to write
Branch (in the "To" block)Your servicing LIC branch, the one that issued or now services the policy
(1) Policy Nos.Up to six LIC policy numbers, one per row of boxes [1]
Name of Policy holder / claimantExactly as it appears on the policy, and matching the bank account name [1]
(2) Bank Name and (3) Bank Branch AddressThe bank and branch where the account is held
(4) Account TypeSavings, Current, Cash Credit or NRI. NRE accounts carry extra conditions [1]
(5) Account No.Written from left to right in the boxes, as the form instructs [1]
(6) IFS CodeThe IFS code of your branch, as on the cheque leaf or passbook page you attach
(7) Mobile NumberYour number, after the pre-printed +91 [1]
(8) E-mail IDYour e-mail address
(9) SMS / e-mailTick Yes or No for messages on matters related to your LIC policies
(10) Aadhaar, (11) PANThe numbers; self-attested copies of both are on the enclosures list [1]
(12) Passport NumberMandatory only if the account type is NRE [1]
Date and signatureSignature of the policy holder/s, at the foot of page 1

A few things the boxes don't make obvious. The account number goes in left to right, starting in the first box [1]. The name line is where most mandates go wrong: page 2 says the name under the policy should match the bank account name, "else it is likely to be rejected" [1]. If your policy says "Sunita R. Sharma" and your passbook says "Sunita Sharma", sort that out with the bank or the branch before you submit, not after the maturity date has passed.

The same note asks you to check with your bank that the account is NEFT-enabled, and to keep it operational until the payment arrives [1]. A dormant savings account that hasn't seen a transaction in two years is the kind of thing that defeats an otherwise correct form.

4. What to attach

The tick-boxes at the bottom of page 1, in the form's own order [1].

ItemAttachWhen
(a)Cancelled cheque leafAlways, if your cheque carries your name
(b)Photocopy of the bank passbook page with account number and IFS codeIf the cheque does not show the account holder's name
(c)Self-attested photocopy of Aadhaar cardListed for everyone
(d)Self-attested photocopy of PAN cardListed for everyone
(e)Self-attested photocopy of passport, plus a bank certificate that premiums were paid through the NRE accountOnly for an NRE account [1]

Page 2's English note (c) reads slightly looser than the tick-list: it asks for "either a cancelled blank cheque leaf or the photo copy of the page of the passbook/cheque book" with the account details [1]. A cancelled cheque with your name printed on it is the first item LIC lists, and the only one that proves both the account and the name in one piece of paper.

The Aadhaar and PAN copies should be self-attested, meaning signed by you on the photocopy [1].

5. Where to submit, and what happens next

Hand or post the form to your LIC branch, at the address you filled in, or to a customer zone [1]. The form gives no processing time. What it does give is a deadline for chasing: if the amount isn't in your account within two days of the due date, contact the branch office [1]. Keep a photocopy of the signed form and its enclosures, with the date you handed them over.

If you later move banks, the old mandate does not follow you. A fresh form is required for any change in bank details [1].

6. The NRI problem the form doesn't resolve

This is the one place the form contradicts itself, and we can't tell from the document which half LIC applies today. Page 1 offers NRE as an account type and sets out what it needs: a bank certificate or certified statement showing the policy money flows through the NRE account, citing RBI's FEMA 2000 guidelines, your passport number, and a declaration that premiums were paid from that account [1]. Page 2's note (h) says the opposite: LIC "has decided not to include NRI accounts for fund transfer", and NRIs should not submit their NRI account details [1]. If you are an NRI with a maturity coming up, ask the servicing branch in writing which rule it follows, and do it well before the due date.

7. The other LIC forms people need alongside it

The NEFT mandate only says where to pay. The claim itself is a separate form, and LIC publishes each one on licindia.in/download-forms. We host only the NEFT form; these links go to LIC's own copies.

  • Maturity: the Discharged Receipt, Form No. 3825. LIC says the branch usually sends a maturity intimation two months ahead, and asks for Form 3825 with the original policy document at least one month before the due date [2].
  • Survival benefit (money-back instalments): Form No. 5180 [2].
  • Death claim: Claim Form 'A', Form No. 3783, or 3783A if the policy has run three years or more from the date of risk [2].
  • Pension and annuity payments: the separate IPP-NEFT mandate form, addressed to the IPP Cell rather than a branch [2].
  • Paying premiums from your bank is a different direction of money and a different form: the ECS Direct Debit Mandate [2]. Don't send it in place of the NEFT mandate.

Our view: if a maturity or money-back payment is due within the year, send the NEFT mandate now, on its own, rather than bundling it with the claim at the last minute. A rejected mandate found early costs nothing; one found after the due date costs you the wait.

8. What the form does not tell you

It gives no processing time for the mandate, and no way to check that it has been registered. It does not say whether a mandate already on file from an earlier claim is reused. It sets no deadline for submitting it, beyond the two-day chase rule [1]. And it says nothing about joint accounts, beyond "Signature of the policy holder/s". For any of those, the servicing branch is the only authority the form names.

Where the rules may be heading: IRDAI's September 2026 draft on insurance distribution proposes that claims be paid only into the verified account of the policyholder or nominee [3]. It is a draft, covered on our page on the IRDAI consultation paper, but it points the same way as this form's name-match rule.

Frequently asked questions

Where can I download the LIC NEFT form PDF?
Here: LIC NEFT mandate form (PDF, 1.3 MB), an unchanged copy of the file LIC publishes at licindia.in/documents/d/guest/neft_mandate_form. On LIC's own site it sits under item 12, "Neft Mandate Form", on licindia.in/download-forms [2]. LIC's page labels it 58.7 KB. The real file is 1.3 MB because it is a scan [1,2].
What documents are required for the LIC NEFT form?
A cancelled cheque leaf, or a photocopy of the passbook page showing your account number and IFS code if the cheque doesn't carry your name. The form's tick-list also asks for self-attested photocopies of your Aadhaar and PAN cards, and of your passport if the account is NRE [1].
Where do I submit the LIC NEFT mandate form?
To your LIC branch, at the address you write in the "To" block, or to an LIC customer zone [1]. The form itself mentions no postal or email route beyond that.
Can I use one NEFT mandate form for more than one LIC policy?
Yes, for up to six. Page 1 has boxes for six policy numbers and page 2 says the mandate "can be used for 6 different policy numbers" [1]. A seventh policy needs a second form.
Is the LIC NEFT form needed for maturity payment?
The NEFT mandate is how you give LIC the account for "receipt of policy payment through NEFT" [1]. It is not the maturity claim itself: that is the Discharged Receipt in Form No. 3825, which LIC asks you to submit with the original policy at least one month before the due date [2]. Sending both together saves a second trip.
How do I change my bank account for LIC payments?
Submit a fresh NEFT mandate form with the new account details. Page 2 says any change in bank details after submission needs a new mandate [1].
Can an NRI receive LIC maturity money in an NRE account?
The form is inconsistent on this. Page 1 lets you choose NRE, asking for a bank certificate, your passport and a confirmation that premiums were paid from that account [1]. Page 2 says LIC has decided not to include NRI accounts for fund transfer and asks NRIs not to submit them [1]. Ask your servicing branch which applies before relying on either.
